COVID-19 NOTICE & UPDATES
Dr. Doko will be offering telephone visits and in-person visits.

Dr. Doko's Office will call you if you are eligible for a
COVID-19 Vaccine based on Ontario's Vaccine Eligibility. More information about vaccines will be coming soon as we have more details from the Health Unit.
In-Person Visits
Hours for in-person visits will be Monday, Tuesday, and Thursdays 9 am to 1 pm. Visits will be booked if approved by doctor. We are minimizing in-person visits to only essential visits approved by doctor.
When you arrive at the clinic:

Remain in your vehicle or remain outside the clinic and call the phone number

Call 519-254-7979 ext 2 and Staff will check you in if doctor approves your in-person visit

You will be called to enter the clinic when room available

Maintain social distance with others

Please sanitize your hands with sanitizer provided

You must wear a mask or face covering

Each room is disinfected after each patient visit

Please maintain social distancing while in the clinic

If the visit is for a child, 1 adult can accompany the child

Children under age 5 are not required to wear a mask in the clinic
If you are arriving for an in-person visit then you confirm that you have not had close contact with a confirmed case of COVID-19.
If you need to see the doctor and you have fever and/or cough please let the Staff know at check-in and further instructions will be given.
The walk-in visits will be first come, first serve but visits must be approved by doctor. Please note that due to volume of patients the doctor may stop taking patients earlier than expected.

THERE IS A COVID-19 ASSESSMENT CENTRE OPEN AT OUELLETTE HOSPITAL DOWNTOWN, ENTRANCE IS NEAR THE EMERGENCY ROOM.
